After completing the forms either: (1) Fax to (702) 938-5844; or (2) email to: droyal@royalmedicalclinic.com; and (3) Pay for Test ($160 insurance co-pay⁺ or $320 cash pay) and Administrative Fee ($100). 

NOTE: After completing the above, you will receive your test kit for blood draw and requisition form.  Results take up to 2 weeks to complete after being received by lab.  Once results are available, you will then receive instructions on how to proceed with supplement.

*Participant may receive assessment in NV, UT, and WI, but must otherwise obtain a referral from physician for "homeopathic consultation".
